Privacy Policy

This policy applies to information collected through this website, operated by Madison Eye Care Center, 66 Hughes Road, Madison, AL 35758. It does not describe how information you give us in the office, by phone, or as part of your care is handled — ask the front desk for the practice's in-office privacy notice.

Please do not submit medical details through this website. The forms here are for scheduling and general questions. If you need to discuss symptoms, test results, or anything about your care, call us at (256) 461-7100.

We only collect what you type into a form or what your browser sends automatically.

• Appointment request form: name, email address, phone number, whether you are a new or returning patient, the reason you selected for the visit, your preferred date and time window, and any optional notes you choose to add.
• Contact form: name, email address, optional phone number, subject and your message.
• Optional marketing consent: if you tick the optional boxes, we record that you agreed to receive practice emails and/or text messages, and the wording you agreed to.
• Technical data: our hosting and backend providers process standard request data such as IP address, timestamp, browser type and pages requested for security, error handling and abuse prevention. We keep short-lived submission records (a hashed identifier and timestamp) to rate-limit spam.

We do not ask for Social Security numbers, payment card or bank details, insurance identifiers, or diagnosis information through this website. Please do not send them.

• To respond to your message or call you back to schedule an appointment.
• To send you a confirmation copy of what you submitted.
• To keep the site working, prevent spam and abuse, and diagnose errors.
• To send practice news or appointment-related marketing only if you separately opted in.

Submitting a form never requires you to agree to marketing. Consent to marketing is not a condition of receiving care or of using this site.

We use a small number of vendors to run this site. They process information on our behalf under their own terms and privacy policies:

• Website hosting and database (Lovable Cloud, built on Supabase): stores form submissions and serves the site.
• Transactional email delivery: sends the notification to our office and the confirmation copy to you.
• Google Maps (optional): the directions panel on our Contact page loads from Google only if you allow it in your cookie preferences.
• ElevenLabs (optional): the virtual assistant widget loads from ElevenLabs only if you allow it in your cookie preferences. Anything you type or say to the assistant is processed by ElevenLabs.
• Google Fonts and a public script CDN: used to deliver typefaces and the assistant's loader script; these providers receive your IP address as part of the request.

We may also disclose information if required by law, to protect our rights or safety, or in connection with a sale or reorganization of the practice.

Website form submissions are kept for as long as needed to answer you and to maintain a record of the request, then removed on a periodic basis. Anti-spam records are short lived. Ask us if you would like a submission deleted sooner.

• Cookies and third-party embeds: use Cookie settings at any time, or read the Cookie Policy.
• Global Privacy Control: if your browser sends a GPC signal, we treat it as an opt-out and leave every non-essential feature off unless you turn it on yourself.
• Marketing: unsubscribe from any marketing email, reply STOP to any text message, or contact us and we will remove you.
• Access, correction or deletion: email info@madisoneyecarecenter.com or call (256) 461-7100 and we will respond as required by applicable law. We may need to verify your identity first.

We validate and limit what forms accept, transmit the site over HTTPS, restrict access to submitted data to the practice and its service providers, and keep dependencies patched. No website can be guaranteed completely secure, and we do not promise that transmissions over the internet are risk-free.

We care for patients from six months of age, and a parent or guardian may submit a request on a child's behalf. This website is not directed to children, and children should not submit forms themselves.
